BCIC investigation blames negligence for gas leakage
The 10-member committee formed by Bangladesh Chemical Industries Corporation (BCIC) submitted the primary report on the gas leakage of Di- Ammonia Phosphate (DAP) Plant on Sunday.
Although BCIC chairman Mohammed Iqbal did not disclose the report saying that the minister concerned would talk about the accident to the press, sources said, the report blamed negligence and mismanagement of DAP as the prime cause of the accident occurred on August 22.
Sources said, two committees were formed to investigate the occurrence of leak of pipe lines of tank of Di- Ammonia Phosphate (DAP) Plant at nearby area of Chittagong Urea Fertilizer Company Limited (CUFL). The committees visited the spot last week. The 10-member committee was formed by Bangladesh Chemical Industries Corporation (BCIC). The committee is leading by Director of BCIC (Technical and Engineering) Ali Akkas as convener and Managing Director of Chittagong Urea Fertilizer Company Limited (CUFL) Abu Taher Bhuiyan as member secretary. The committee has submitted the report to authorities concerned on Sunday evening, said sources.
